Analysis of Application of Enteral Nutrition in Children with Critical Illness on Extracorporeal Membrane Oxygenation

Abstract:

Background: To analysis the status of Enteral Nutrition (EN) among children under ECMO and influence factors on EN. Methods: Patients receiving ECMO and EN admitted to PICU were selected. Everyone was recorded with baseline characteristics, items of EN and levels of GAS, MTL and Ghrelin. Fifty critical illness children without ECMO were chosen as control. Results: Forty-eight children receiving ECMO were enrolled in the study. Children achieving EN within 24 h after ECMO initiation were 68.8%. The most common episodes during EN were gastric retention, and intestinal necrosis and hemorrhage was not found. Across the first week after ECMO running, energy from EN increased slowly in general. Afterwards, EN intake began to decrease until weaning of ECMO. Children under 1 year old were provided with the highest calorie during ECMO support. As withdrawal of ECMO, energy from EN increased significantly in children under 1 year old and 1 to 5 years old. The concentrations of GAS, MTL and Ghrelin decreased significantly after 5 days of ECMO running, then increased after 2 to 5 days of weaning. Energy from EN was related to the severity of underlying diseases and use of vasopressor. Energy by EN in ECMO patients decreased more evidently three days after ECMO start than children without ECMO. Conclusion: Early EN was tolerable and safety for children under ECMO. The energy from EN in ECMO children increased more slowly than children without ECMO. EN intake was related to the severity of underlying disease and use of vasopressor.
