Serous Borderline Ovarian Tumor Recurrence with Upgrading to Low Grade Serous Carcinoma Many Years after Initial Diagnosis: A Case Report and Review of the Literature

Abstract:
Introduction: Over 90% of serous borderline tumors are discovered at an early stage. Most patients diagnosed with serous borderline ovarian tumors have an excellent prognosis, as estimated 5-year survival is above 95% regardless of stage. Recurrence is rare in borderline tumors, with most recurrences occurring during the first 2 years and only 1% to 2% upgrading to invasive Low-Grade Carcinomas (LGSC). Case Report: The current report describes a case of a 55-year-old woman who was diagnosed with borderline tumor, underwent surgical treatment and adjuvant chemotherapy. 17 years after the initial treatment, she was diagnosed with recurrent disease on the vaginal cuff, which was upgraded to invasive carcinoma based on histology. She underwent repeat surgical resection, and after careful consideration, was treated with adjuvant brachytherapy and hormonal treatment. Discussion: This report presents the rare case of borderline ovarian tumor that recurred after many years of remission and upgraded to invasive carcinoma in a patient with no risk factors for recurrence at initial presentation. Previous reports have discussed recurrence and upgrading of borderline ovarian tumors, but none thus far have reported on recurrence after 17 years of remission in a patient with no risk factors for recurrence. This report also provides a review of the current literature regarding this rare phenomenon.
