Laparoscopic Sigmoidopexy as a Temporizing and Minimally Invasive Strategy in a Frail Patient with Sigmoid Volvulus: A Case Report

Abstract:

Introduction: Sigmoid volvulus is a common surgical emergency. Although sigmoid resection is considered the definitive treatment, emergency surgery may be associated with high morbidity and mortality in frail patients. Alternative strategies may be required in selected cases. Case Presentation: We report the case of a 60-year-old tetraplegic woman with severe psychomotor retardation admitted for acute colonic obstruction due to sigmoid volvulus. Endoscopic detorsion failed. Considering the patient’s extreme frailty, laparoscopic sigmoidopexy was performed as an emergency procedure, with the aim of overcoming the acute phase and planning delayed definitive surgery. Discussion: Sigmoidopexy does not eliminate the pathological segment but may represent a useful temporizing option in high-risk patients. The laparoscopic approach offers the benefits of minimal invasiveness and faster recovery. Conclusion: In selected frail patients, laparoscopic sigmoidopexy can be considered as a bridge strategy before delayed sigmoid resection.
